Introduction to MyChart SolutionHealth?
MyChart SolutionHealth is your personal health connection to your medical record at Elliot Health System and Southern New Hampshire Hospital. SolutionHealth refers to both Elliot Health System, and Southern New Hampshire Health System. You can access portions of your electronic medical record via your Internet browser or through Apple and Android devices.

MyChart SolutionHealth is the primary mode of communication between you and your provider offices related to test results, prescription refill requests, appointment requests, messaging your healthcare providers and their staff, keeping track of important healthcare issues, and review other important aspects of your healthcare.

MyChart SolutionHealth is used across the Elliot Hospital and Southern New Hampshire Hospital to share information with our patients.

    What can I do on MyChart SolutionHealth?

    Message your Elliot or Southern New Hampshire Hospital provider’s office
    Click the messaging icon at the top of the page or click “Ask a question” under Quick Links.
    Request a prescription refill
    Click the Messaging icon at the top of the page or click “Refill Medications” under Quick Links.
    Request a referral
    Click the Messaging icon, select “Request a Referral” under Quick Links.
    Receive and print letters from your provider’s office
    Click the Messaging icon at the top of the page. Click Letters, click on any letter to open it. (Print icon is in upper right corner).
    Request Medical Records
    Under Quick Links, choose “Request Medical Record” Or click here to log in and access the form: Request Medical Record
    Request Radiology Images
    Under Quick Links, choose “Request Radiology Images” Or click here to log in and access the form: Request Radiology Films
    View/print immunization records
    Choose Health tab at the top of the page and click Immunizations. (Print icon is in upper right corner).
    View/Download/Transmit summaries from office visits and hospital visits
    Choose Health tab at the top of the page and click Download My Record, choose Visit Record, select the visits you wish, and either choose to view, download or send to the email address of your choosing.
    View/print test results
    Choose Health tab at the top of the page and click Test Results, Check off “Show Hospital Results” to display any hidden tests. Click on any test to view the result and comments from your doctor. (Print icon is in upper right corner).

    *SolutionHealth publishes test results to MyChart SolutionHealth immediately when they are resulted. We currently do not publish HIV or pathology, however your provider may share them manually, or you can request them electronically Request Medical Record

    *We publish test results from SolutionHealth providers as well as results from tests ordered by providers outside of our organization for your convenience.

    What information can I view in SolutionHealth MyChart

    You can view upcoming and past appointments, medications, health history, care team and billing information.

    As of July 6, all diagnostic lab test and imaging test results, as well as clinical notes from outpatient appointments and inpatient stays, are immediately released and able to be viewed in MyChart, with some rare exceptions.

    When can I see my lab/Imaging test results?

    We share most lab test and imaging results with you immediately as they become available through SolutionHealth MyChart. Exceptions to the automatic sharing are HIV results and Pathology results, which can be requested. Since results are available immediately, you may see test results before your provider has seen them. Please be assured that your provider looks at every test result and will contact you if any follow up is needed. If you have any questions about test result, do not hesitate to reach out to your provider’s office.

    Will I be notified when new information is available to view

    To reduce the number of notifications you receive, we will not send email or text notifications when new test/imaging results are available. We also do not send notifications when new notes are signed by your provider.

    Update your address, phone, and email

    Click Profile tab at the top, choose “Personal Information” make desired changes and click "Accept Changes" to update your medical record. The change reflects immediately in your health record, and your provider’s office receives notification that you made changes.

    I see a non-SolutionHealth provider, how can I see my test results from testing done at Elliot or SNHH?

    SolutionHealth automatically shares results with patients for tests ordered by non-Elliot/non-SNHH providers. Exceptions are HIV and Pathology which are not automatically shared, but can made available upon request.

    How can I set up access for myself?

    Any SolutionHealth patient can have a MyChart SolutionHealth account. Please ask your SolutionHealth providers office to send an activation code to your email address. Or, you may visit MyChart.SolutionHealth.org and click the "Sign Up Now" option found just under the logon box.

    You can also email MyChartSupport@solutionhealth.org and ask to sign up.

    * If emailing your request, please include your date of birth, full name including middle initial, the name of your SolutionHealth provider and your phone number in case we need to verify additional information.

    How do I get access for my child?

    Parents of children age 0-11 can have full access to their child’s account. You need to sign and return the Proxy Access Form. Once Complete, please send completed document to the appropriate medical records location indicated at the bottom of the form and we will set up your proxy account. Once your child turns 12 access will automatically terminate and you will need to sign a new form for new limited access.

    Parents of children 12-17 years of age can have limited access. You will need to fill out a new Proxy Access Form. Once Complete, please send completed document to the appropriate medical records location indicated at the bottom of the form and we will set up your proxy account. Once your child turns 18 access will automatically terminate.

    *Teens 12-17 are able to have access to their own MyChart SolutionHealth accounts.

    How do I set up access for another adult? (Anyone 18 or older)

    The only proxy access that will be created directly by a workforce member of SolutionHealth will be for adult(s) who are the legal guardian of the patient(s). If you are 18 years or older and would like to grant another competent adult proxy access to your MyChart SolutionHealth account, you must follow the steps outlined below in Paragraph 3. SolutionHealth will not be responsible for directly establishing these types of accounts and the responsibility will be on you to grant another individual(s) proxy access to your patient portal account. Court appointed legal guardian(s) may request proxy access to another individual’s MyChart SolutionHealth account. To obtain proxy access the legal guardian must fill out and submit the MyChart SolutionHealth Proxy form. You further agree that in the event you request proxy access to an individual(s) account you will be required to provide documentation of your authority (for example, court documentation you were appointed guardian) before you can receive an access code to complete registration or access that individual(s)’s personal health information. Access to another adult’s account is good for 365 days, at which time your access will terminate, and you will need to complete the MyChart SolutionHealth Proxy form and certify you are still the individual(s) appointed legal guardian. SolutionHealth reserves the right to revoke proxy access at any time.

    Adult Patients (18+) - Other Proxy Access

    Individual(s) 18 years or older may grant another individual(s) proxy access to their patient portal directly through the patient portal. If you would like to grant another individual(s) proxy access to your patient portal, you may do this by going to the “Health” tab, clicking “Share My Record”, then choosing “Friends and Family Access”, click “Invite Someone”, and fill out the appropriate information to send the individual(s) of your choice an invitation for access to your health record. The individual(s) you invite MUST be a patient of SolutionHealth. The proxy access for this individual(s) will last for 365 days. If you want to terminate this individual(s) proxy access you MUST notify your provider. You understand and agree that you are solely responsible for the accuracy of information you enter into the patient portal and the selection of the appropriate proxy individual(s) when sending a proxy access invitation. You further agree that if you grant proxy access to other individual(s) through this process you release SolutionHealth from any legal responsibility of liability relating to the access and release of your protected health information to the individual(s) you granted proxy access to.

    *Access to another adults account is good for 365 days or 1 year. Once expired, new forms must be signed and returned.

    Scheduling appointments online.

    *If you are experiencing a medical emergency, please dial 911. Online scheduling is only to be used for non-emergent visits.

    Click “Visits” at the top, choose Appointments and Visits. From Quick Links choose “Schedule an Appointment”. You can schedule either your yearly Physical, or a Follow-up Visit. You can click Request Other Appointment to send a message to the office for any other type of appointment.

    *You may only schedule appointments online with providers you have seen before. This Feature may be temporarily unavailable due to COVID19 scheduling restrictions.

    Cancelling and Rescheduling Appointments.

    From “Visits” tab at the top, click Appointments and Visits. Choose the upcoming visit you wish to cancel or reschedule. Click to open it, choose appropriate option, fill out information and click send. Your provider’s office will receive notification of your cancellation or rescheduled visit.

    *You will not see the option to cancel appointments that are within the next 24 hour period. For appointments this soon, please call the office directly to cancel.

    How to correct inaccurate information.
    The information on MyChart SolutionHealth comes directly from entries made inside your SolutionHealth medical record. If you see something that is incorrect, please send a message to your provider’s office reporting the inaccuracy and ask them to correct it. To send a message, click “Messaging” at the top of MyChart SolutionHealth. You may also phone the provider's office directly and ask them to correct the information.

    Sharing email addresses.
    Because your email is your MyChart SolutionHealth username, you cannot use the same email address to sign up multiple family members.

    Downloading the MyChart SolutionHealth Mobile for iPhone and Android.

    Follow the steps below to download and log into the MyChart SolutionHealth mobile application.

    Note: You must have a MyChart SolutionHealth account before logging into the mobile app.

    • Download the free MyChart Application from the iTunes App Store or Google Play Store (search for “Epic MyChart”).
    • Select the MyChart Application
    • Accept the Apple or Android License Agreement
    • Select your location (New Hampshire).
    • Select MyChart SolutionHealth from the list of available health care providers.
    • Log in using your MyChart SolutionHealth account credentials.

    • *While the MyChart SolutionHealth mobile app includes variations of MyChart SolutionHealth features, it does not replace the web-based version of MyChart SolutionHealth. There are certain features that patients can access only in the web-based version.

      Minimum Technical Requirements.
      You need access to the internet and access to a computer. You need access to a browser: Internet Explorer 9.0 (standards mode and IE8 compatibility mode), Internet Explorer 8.0 (standards mode only), Mozilla Firefox 13.x and above, Safari 5.x on Mac/Windows, or Google Chrome 20.x and above.
